Regulated Facilities
SQG, VSQG, and HW Transporter Closure
Small Quantity Generator, Very Small Quantity Generator, and HW Transporter Closure may request closure of their EPA Identification Number.
Audits
LDEQ's voluntary environmental self-audit program was established to enhance protection of human health and the environment and increase environmental compliance at regulated entities.
HW-1 Form
Large Quantity, Small Quantity, and Very Small Quantity Generators and Transporters of Hazardous Waste are required to submit the Louisiana Notification of Hazardous Waste Activity (HW-1) Form to notify LDEQ of regulated hazardous waste activities. Also, Large Quantity Generator and permitted Treatment, Storage or Disposal Facility (TSDF) may use the HW-1 for to submit their required Annual Hazardous Waste Report.
Solid Waste Operator
The Application for Solid Waste Operator Certification is used when an individual is seeking to become an operator at a solid waste processing or disposal facility.
Generator of Industrial Solid Waste Forms
This notification form is to be submitted to the LDEQ by any person whose act or process produces industrial solid waste as defined in LAC 33:VII.115.A.
Transporter of Solid Waste Forms
This notification form is to be submitted to the LDEQ by any person who moves solid waste off-site to a non-processing transfer station or a collection, processing, or disposal facility as defined in LAC 33:VII.115.A (excluding individuals who transport their own residential waste to a collection facility, non-processing transfer station, or permitted processing facility and/or solid waste landfill).
Solid Waste Notification Form for Solid Waste Collection Facilities, and Non-Processing Transfer Stations, and Best Management Practices Plans.
This notification form is to be submitted to the LDEQ by any person who operates a solid waste collection facility, non-processing transfer station, best management plan or other facility requiring notification under LAC 33:VII.303 or 305.
Industrial Solid Waste Generator Annual Report Form
Industrial Solid Waste Generators are required to submit an annual report by August 1 for the previous reporting year (July 1st through June 30th).
Non-processing Transfer Facility Annual Report Form
Non-processing Transfer Facilities that separate non-putrescible commercial solid waste for recycling are required to submit an annual report by August 1st for the previous calendar year (July 1st through June 30th).
Certification of Compliance (Annual Report)
All permitted solid waste facilities are required to submit a Certification of Compliance (annual report) by October 1 of each year covering the previous state calendar year (July 1st through June 30th).
Waste Tire Generator and Motor Vehicle Dealer Forms
Waste tire generators are regulated under LAC 33:VII.10519 and Motor Vehicle Dealer are regulated under LAC 33:VII.10521 are required to submit monthly reports by the 20th of the following month.
Waste Tire Transporter & Transfer Facility Notification Forms
Waste Tire Transporters and Transfer Facilities are required to notify LDEQ to receive valid transporter authorization.
Waste Tire Single Event Clean-Up & Government Tire Sweep Forms
Waste tire single event clean-up and government tire sweeps are regulated under LAC 33:VII.10529.
Government Waste Tire Collection Center Forms
Government agencies that want to run a waste tire collection center or processing facility must notify the the department before they begin operating. These government‑run collection centers and processing facilities do not have to get a standard waste tire collection center permit as long as they follow the rules listed in LAC 33:VII.10525.J (processing facilities) and LAC 33:VII.10527.H (collection facilities).
Waste Tire End-Use Project Application Forms
End-Market Use Projects are a project that uses whole waste tires or tire material in a way that has been approved by the state. This means the tires are reused or recycled for a specific purpose that meets safety and environmental rules.
Waste Tire High Volume End-Use Facility Forms
A High Volume End‑Use Facility is a place that uses whole waste tires or tire material for large projects that need long‑term storage. These projects must be approved by the state. This type of facility can also include ports where tires are stored for a longer time before being shipped by boat to approved end‑use projects in other states or other countries.
Waste Tire Processor Forms
Waste Tire Processors are required to submit monthly reports to LDEQ by the 12th day of the month. These reports document the movement of tires and disposal of the waste.
Annual Recycling Report Form
LDEQ collects and compiles submitted information and data provided by parishes and municipalities on resource recovery and recycling programs in their area under La. R.S. 30:2413.
Emergency Debris Site Forms
Local Government and Parishes must request and receive approval for emergency debris sites, request activation once and emergency is declared, submit weekly reports detailing the amount of waste managed and request deactivation once the emergency event ends or the location is no longer needed.
