Hazardous Waste Notifications

Notification

Large Quantity, Small Quantity, and Very Small Quantity Generators and Transporters of Hazardous Waste are required to submit the Louisiana Notification of Hazardous Waste Activity (HW-1) Form to notify LDEQ of regulated hazardous waste activities.

The HW-1 Form includes guidance on who must submit the form and regulated activities that require a notification.

Closure Notifications

Use the LDEQ HW-1 Form or EPA Site Identification Form (EPA 8700-12) to submit notifications to LDEQ. Submit a cover letter with the notification detailing the reason for submittal.

Both forms do not have to be submitted as the HW-1 Form & EPA Form 8700-12 are interchangeable.

Annual Reports

Large Quantity Generator and permitted Treatment, Storage or Disposal Facility (TSDF) are required to submit an Annual Hazardous Waste Report. Reports can be submitted electronically or by paper forms, both submittals are not required. LDEQ does encourage the use of the EPA RCRAInfo Reporting software to complete and submit annual reports.

The 2025 reporting year is a Federal reporting year. 

Electronic Submittal

Electronic reports should be submitted using the EPA RCRAInfo Reporting software.  Paper reports should be submitted to the Louisiana Department of Environmental Quality using the EPA Hazardous Waste Report Instructions and Forms Booklet and the data will be entered into the EPA RCRAInfo Reporting Software.  

All generators reporting electronically need to register with RCRAInfo. Go to this link https://rcrainfo.epa.gov/rcrainfoprod, click on “REGISTER” and select “INDUSTRY USER”. This will prompt you to create a User ID and Password, and then there will be 3 security questions to activate your login. If you have credentials for EPA’s Central Data Exchange (CDX) or have already created an account for the RCRAInfo Industry Application, enter those credentials in the URL specified. (Click here for help: New Industry User Help)

The EPA Hazardous Waste Report Instructions and Forms Booklet provides information on completing your 2025 Hazardous Waste Report. Paper reporting forms may be found at the end of the instruction booklet.
