Request Form Submission:

All request forms shall be automatically emailed to the Louisiana Department of Environmental Quality (LDEQ) at DEQ-CSV@la.gov.  All submissions directed to DEQ-CSV@la.gov will receive automatically generated emails acknowledging receipt.  If this automated confirmation email is not received within 2 business days after electronic submission, call 225-219-3786 to verify receipt of electronic submission with LDEQ personnel. 

Compliance Certification emails will be issued from LDEQ staff within 20 days of receipt of the request form once documentation provided has been reviewed for completion and staff has identified if standards of compliance under Act 144 have been met by the applicant.

Act 144 and shall not be construed to preclude the right of LDEQ to conduct a comprehensive compliance evaluation in accordance with the Louisiana Environmental Quality Act, and to pursue any enforcement action and penalty associated with any violations discovered as a result.
